Students interested in being considered for the ISP program after the deadline must contact their school counselor directly.

- Current Independent Study students who plan to continue in the ISP program need to complete and submit this enrollment form to continue for the 2025-26 school year.
- Current Independent Study students who plan to return to in-person learning do not need to complete this form.
NOTE: This is not the form for Independent Student Physical Education (ISPE). Please contact your middle or high school for information on this program for 2025-26. Please complete this application form ONLY if you are interested in enrolling your child in SMMUSD's Independent Study EDUCATION Program for the 2025-26 school year.

Information Sessions: Information Sessions on the Independent Study program will be held in May and August.
Written Agreement: A key component of the enrollment process is a signed written agreement that must be obtained from the parent/guardian. This will be completed during an in-person meeting and includes the student, parent, counselor, ISP teacher, and school administrator.
